Put in the email address you were contacted at and it comes off the list. You do not have to give a reason, log in, or confirm anything afterwards.
What happens next. Removal requests are handled by hand, usually the same day and always within three business days. That is well inside the ten business days the law allows. Removal is permanent. I also suppress the business name and, for a company domain, the whole domain, so a colleague's address at the same company will not be contacted either. Addresses at personal mailbox providers such as Gmail or Yahoo are suppressed by address only.
The record is kept permanently. That is the only way to guarantee you are never contacted again by accident, and it is the one piece of information I will not delete on request, because deleting it would defeat the purpose.
Replying to any message from me works exactly as well as this form. Any wording counts.
